This is for someone with 2 or more chronic conditions requiring regular monitoring, hormone therapy, or those with specific health goals requiring more frequent monitoring. This tier includes annual physical exam, 3 follow up exams (typically quarterly), and up to 5 sick/additional visits per plan year. Join fee $189
Functional medicine in depth physical and lab assessment. This tier is for those who have tried everything to solve their health concerns or those who are health-conscious with specific athletic or long-term health goals in mind. This tier is anti-aging and longevity based focused on addressing the root cause of illness and disease. Includes annual physical exam, in-depth functional health lab assessment, 3 follow up exams, and up to 12 additional visits per plan year. $500 join fee.
