60-min functional medicine visit focused on your primary concerns. If you have multiple concerns, we recommend membership based or package care to ensure your needs are met.

- 90-min Specialty comprehensive annual review and functional health planning visit
- Quarterly (3) 40-min provider follow up visits per year
- Discounted lab options
- Lab interpretation including advanced testing and advanced biomarker tracking
- Care coordination
- Unlimited secure messaging
- Plus up to 8 additional as-needed visits per year
- Can be billed annually or monthly - $120/month
Includes:
- 60-min Initial Evaluation for bioidentical hormone replacement therapy, hormone optimization, thyroid, or metabolic health optimization
- 3 provider follow up visits within 1 year for the same goal.
